Most of the time I finish the review and pick the title last. I want it to reflect the tone of my writing which I can't always do at the beginning. That said, my titles are rarely as comedic as other reviewers : (

Yes, sometimes I have trouble but most of the time, coming up with a funny or amusing title is rather easy.
