The list you have is great and I think its so fantastic that you are taking the time to get back to being you and putting you first. On top of life/personality/thing s that will make me better goals, every year on my birthday I make a list of things I want to do that year (think bucket list but instead of having your life to do them, you have 12 months). I find by keeping the list short and re-editing it annually it is more achievable and not as overwhelming as a "100 things to do before I die" sort of list.
I try to use the list for exciting opportunities that I hope to have, know I may have or will make for myself during the next 12 months. Usually the list is between 5-10 items; sometimes including simpler to accomplish items, some that may only be a dream and some in between.
For example, a few years ago I went on a trip and knew I would be going on a sea cruise where I would have the opportunity to swim with sting rays. Now I am terrified of sting rays but was struggling between finding the nerve to get in the water and passing up the fantastic chance that I would have; so I put it on my list on my birthday to motivate me (I did get in the water and pet one stingray for about 5 minutes before swimming as fast as I could back to the boat).
Some years I manage to cross more off the list, others I don't do as well and it creates a fire in my belly for the next year. This year I'm not doing so well sticking to my list, money has been an issue (as well as time) but I hopefully can turn the around in the remaining months.

PS. this year two of my list items are (1) to plan a trip back home (shouldn't seem so difficult but I haven't been back in 5 years) and I just booked the flight today (exciting); (2) to learn another language to the point of being able to hold a conversation.
