It is able to be worn in four different settings allowing for a range of sizes. My boyfriend tends to be on the larger end of the scale and a lot of these products don't work for him yet we can fit him in the second to largest setting. The only sizing issue I could see is that it tends to twist on the smallest possible setting, so it may cause extra chafing on smaller guys.
