A nice idea, but too large for us
Overall, this is a good idea, but it's just not for us. Your mileage may vary, based on head size and mouth size.

On the surface, it seems like a great beginners' gag. I'm sure it is great, for the people that it fits. For my wife, it was too big to even get into her mouth. I wasn't able to keep it in terribly long before getting jaw fatigue, either. With a 2 inch diameter on the ball, it seems like we're not the only couple for that this has been an issue for. Also, on me, my head (size 7 7/8 hat size) is too large to wear it comfortably. I have to wear it as large as possible, and then the strap cuts into the side of my mouth. If it weren't for that, I imagine that this would be a good way to start with gag play.
There's plenty of holes to allow for breathing, so suffocation won't be a problem. The buckle on the back is secure, so no worries about it slipping off, and the strap should stand up to a good amount of use. Again though, those with bigger heads may find that the strap really cuts into the edge of their mouths. The plastic taste was a bit much for me, too.
Cleanup should be a breeze, as this really shouldn't get exposed to anything other than drool and sweat. The strap being on the back of the head and the full buckle closure means that there's no quick way to take it off, so some form of non-verbal communication is a must if the wearers hands are also restrained. Even though the ball is hollow, it seemed to be fairly sturdy, so I don't imagine a little bit of chewing on it would be an issue. The strap that goes through the ball attaches to the head strap with a rivet on either side; while it may not stand up well to intense use, a beginner should be fine.
If you can handle the size issue, I think that this would be a great gag to experiment with.

As much as I wish we could, we just can't get it to fit comfortably in either of our mouths. It's a shame, as I love the idea behind it.
