I don't think this is true for everyone. It also has to do with materials and sizing, which is why detailed reviews are so wonderful .

My boyfriend has not had pain and he is very girthy.

His friends might have heard of some guy who had a bad experience. We know guys, they "know a guy who knows a guy" lol. Inform him there's so many kinds, you can probably find one that's very stretchy. just to see how he likes it, or those "one time use" ones that super breakable just for that reason.

But it depends more so on the material then the guy. something that doesn't stretch much in the first place shouldn't be on a guy with a lot of girth and vise versa.

The only time I have had pain from one was a leather one, but with something that does not stretch it is easier to put it on to tight. The more flexible/stretchy ones are the more comfortable. In ether case a dab of lube will make things better.

I have had some pain from bruising a time or two from wearing the ring to long, say like an hour or so, but as long as I keep the time down there is no problem. I think with the really tight ones 20-30 minutes is about the max that I would recommend.

Our first non-leather one was bought at Waly-World and came with condoms, the vibe lasted for about 20 minutes or so. Very stretchy and comfy, great way to try one out.

I am curious about how you take the cock ring off after 20 minutes. The one where you put the balls in the ring then the cock after. I am just worried that the cock being hard would make it impossible to remove. What if it takes like an hour to become soft. It makes me a little stressed out, lol. It doesn't take an hour after orgasm for most of us to soften enough to remove the ting. There are many designs - even some that are adjustable for easy removal. Nothing to stress about - let him pick what he thinks will work best from his perspective.

I use cock rings (the stretchy ones) and never had any problem or discomfort ejaculating. Ask him to try it once.
