So I know that people use cock rings either over just the penis shaft or the shaft and the balls. I've never had much luck with using one over the balls as I already have a short penis and putting it behind the balls makes the balls stick out in front. For me it depends on the temperature, that is when it is cooler they tend to be uncomfortable, due to the sack sucking up. When the temp is warmer the sack hangs lower (stretches easier) and then it is way more comfy.
I also find a thinner ring feels better when rapped around the balls, and does not push the boys out as much.

My wife actually likes the fact that my balls are pushed forward because it creates more contact with her, particularly when we're in Missionary. However, I don't find it uncomfortable, so maybe mine aren't pushed as much as what you are experiencing.

The Smokey Elephant has a bullet that is 1/2" in diameter. You could easily put one that is 3/4" maybe bigger. The only thing I can think of is it may change the orientation of the trunk and head of the elephant.
