Without the balls - the dildo can easily rotate inside putting pressure in the wrong places. Since I'm doing this with my wife - I need a frame of reference.

It really depends on the toy, I'm not quite sure what my exact criteria is, to be honest. I do really enjoy some that have balls, but other times the deciding factor in choosing is that it's balls-free. I'm gonna say it's probably more about the overall look of the toy, and I really like having both types in my collection

I generally look first at other things, but balls have their advantages. They provide a good handle and can be used for leverage, and they provide a "this side up" indicator (or more appropriately a "which way is this pointing" indicator). They can get in the way and they definitely make a toy much more realistic (can go either way with that).
