It's a complex interplay of how your body is wired and how you're doing psychologically at the time. To an extent you also shouldn't try too hard, because then you're less likely to get one.
It also takes a while - especially your first time.
The short answer is that the best way to get one is to stimulate your prostate rhythmically in a way that feels good to you. I have tried to have a hands free one also but with no luck. The best luck I've had is trying to find the P spot and using a vibrator there. There are plenty of P spot vibrators around too.
I have read that it takes a lot of practice; It's almost an art.
The key is finding the P spot first and then figuring out how it likes to be stimulated.

It took me over a couple of years to achieve a p-spot orgasm with an Aneros. It was always fun to have in there, and I was able to enhance my traditional ejaculatory orgasms with it, but finally being able to achieve a P-spot orgasm is really amazing. It is nothing like a normal orgasm, and I think the expectation that you are just going to explode like a normal orgasm gets in the way of achieving a prostate orgasm. I've only done it once with a toy (Orgasm + ejaculation), but I've done it maybe 5-6 times without anything (No touching, no toys, nothing at all other than porn and will power), and I've found a few things that you need to know / do.
1) Learn your body, before you even try to do this, learn how to control and then practice and strengthen your pelvic floor muscle. Just google 'pelvic floor men' without quotes and you'll find a few good resources.
2) If you pre-cum, make sure to practice doing these exercises in such a way that it feels good and makes you start to 'leak'. Once you do, you'll have found the right muscles to flex / move - keep it up and narrow down which muscles make you feel the best.
3) Go 2-3x longer than you usually do between masturbation sessions, it's important that you have the build up the first time you do this. It's much much easier and takes a lot less time.
4) Find porn that you enjoy, and is long. Or lots of it. Or better yet a partner who is patient and wants to try something new. I'm talking over an hour. This isn't something that's quick.
5) Get comfortable, relaxed, and worked up. It's important you're as aroused as possible before you start, so whatever works for you - do it. I find that going through my collection of porn for awhile without touching myself does it quite well.
6a) If you're going down the anal route, I don't have much info for you, sorry - the one time I managed to have an actual orgasm AND ejaculation at the same time (They're two separate things, more on this below), I did all of the above, practicing for a few months, flexed the muscles around my prostate and had a fairly strong vibrator pressed directly up again my prostate for about 20 minutes.

Also, and MaoMew touches on this, a hands free orgasm really is a completely different experience than having an ejaculatory one. It simply feels otherworldly and can be a long lasting feeling of unimaginable bliss. I find that after an hour or so of these orgasms and "waves" of pleasure, I usually go for an ejaculatory orgasm because I feel like I am actually done...if that makes any sense. That final orgasm, which obviously involves stimulating my penis, is usually far more powerful than any other ejaculatory orgasm that I experience normally, either through sex or masturbation, even masturbation using a prostate massager of some sort (basically starting out masturbating without the intention of going for an extended hands free session). Understanding that there is a fundamental difference between these types of orgasms and feelings is key to removing expectations that can hinder having a hands free orgasm.
