Script, notes or off the cuff?

When making a descriptive video review, do you plan it out a head of time in anyway or do you just turn the camera on and go?

I always mean to make notes and look at them while I'm shooting, but I never remember and I find that most of the time I don't have much of a problem getting the videos done well enough.

Usually when I do my video blogs, I write myself a script and practice it a few times, because I know that otherwise I will ramble. Now I am trying to do my first review an I have been trying to do it on the fly and I keep getting a case of the "ums". Not sure how to edit them out yet. I am curious to see what other people's process is.
