My wife and I were hooked up by friends.

My boyfriend and I have the worst get-together story! When we met, he was dating a friend of mine while we all worked together. I was dating someone else.

They lasted a little over a week before she wanted to sleep around, and confinded in me how she really didn't like him after all and how he was awful in bed and how he had a small penis and was a terrible kisser.

A month later, after I had been dumped, we started hanging out more platonically. Then one night, we went to a movie that is sort of like a Rocky Horror but different (no dressing up, but interactive audience and lots of drinking)...

We ended up hooking up.

But things went really well from there. So whatever.

Oh, and obviously, I didn't take to heart what my friend said about him. He's the best lover I've had. -eyeroll-

No, any "hook-ups" I ever had stayed that way, despite my sometimes being interested in more. I've found that for solid relationships to form, I've very much had to do the traditional thing where we go on dates but I make them wait for sex. Hook-ups just never called me again, while the guys I made wait were totally drooling after me. Always want what you can't (immediately) have, I guess

EDIT: I suppose that the definition of hook-up matters here. For me it means casual sex. For other posters it seems to mean being set up by mutual friends. I met my current boyfriend through mutual friends but it wasn't an official set-up so I vote no under that definition as well.

I've never transitioned a hook up to a relationship.

When my husband and I first started seeing each other he was in a relationship and I was married. It went on for years b/c we just couldn't seem to stay away from each other. Finally I got a divorce and he was single at the time. He called me up when he found out and we've been together ever since. Now we're happily married.

Yes, 2 times. The first time it lasted 5 years, and this one is looking like it's going to last longer than that. (I'm not sure if that's the norm though.)

I had a hookup that wanted to pursue a relationship with me, but I wasn't interested. He was a really sweet kid, but not very smart.
