Last night the Duke Blue Devils beat the North Carolina Tar Heels 73-68 in their 235th meeting (UNC leads the series 132-103). Certainly, Carolina/Duke is one of the greatest rivalries in US college sports.

I may be biased, but I'll have to say Washington State vs. Washington U is the greatest ever. Could be because I'm about to graduate from one of those institutions...
