As of right now? My sciatica. I've had painful tattoos, but that pain goes away in a set period of time. This pinched nerve comes and goes as it pleases it seems, and has hurt like hell for a lot longer than any of my tattoos.

Ripped scalp, bruised skull, torn cartilege in both shoulder sockets along with stretched tendons and damaged rotator cuffs, collapsed lung, torn lung, bruised lungs, bruised diaphragm, torn kidney, torn spleen and six broken ribs on the same side with one rib broken in four areas and cracked in three more. Numerous deep bruises and lacerations--all at one time from a car accident that was in no way whatsoever, my fault. I was rear ended by a full sized truck that was going 70 MPH. I was in a Ford Escort, and was stopped in the middle turn lane waiting for oncoming traffic to go by so I could turn onto my road, I was going home for lunch and made it there, 2 weeks later!!!

The most painful was having the ovarian cyst burst. I thought I was going to die. The symptoms started the night before, but we did a wait and see kind of thing. I went to bed and woke up the next morning unable to sit up. I was in so much pain that when the nurse came to give me the meds for it, and told me to turn over that I told her to stick it in my thigh. She said no, we argued for a moment and then she dumped it down the drain and tossed the needle into the biohazard bin. I passed out and woke up eight hours later and they still hadn't done anything to me or with me during that time. I was pretty terrified and vowed never to go that hospital again.

Fraction of a second for my prince albert. Glad I do not have to endure child birth!

Going into labor with my first child was the worst, it probably lasted for an hour, and I actually fainted, the doc has to get my baby girl out by vacuum and forceps.
The second most painful thing for me is getting permanent eyeliners, imagine tiny needles poking on those very tender patches of your eyes! But I still have to do it every ten years, because they make my eyes look much brighter, and save me the trouble of applying it every morning.

Wow I had the same thing happen with my son! Three days with no epidural was the worst imaginable. In the end I had my now ex husband out and asked for the epidural...he was not happy

Uh, I'm gonna say when the doctors gave me pitocin (to speed up my labor) without my consent. I was fine until then.

Thankfully I haven't had a whole lot of big injuries (no broken bones or anything), though I've twisted my ankle quite magnificently on more than one occasion. How I manage to not land myself in the hospital sometimes is anyone's guess. I've also gotten a few minor burns here and there.
The only other ones I can think of after that concern my mouth. Once when I got novocaine injected (normally a fairly painless thing), I think my dentist hit a nerve or something because I had really severe pinpoint pain until the drug went into effect. The other would have been after I got my wisdom teeth removed: aside from the recovery pain, my jaw was fairly stiff for a while and hard to open, but at one it point snapped wide open on me for half a second, then went right back. Hurt like hell.

It would be my back spasming up. I injured it and it used to start hurting so bad I would be left on the floor screaming.

Probably menstrual cramps that hurt so bad they made me vomit.
