Doesn't matter to me. The water takes awhile to heat up so usually I start with cold and end with warm.

Warm/hot. My teeth are sensitive and it softens the toothbrush bristles. My mouth rinse is cold enough, I wish I could warm that up.

I used to always use warm water, but a couple of years ago I discovered how refreshing cold water can be!

Depends on how sensitive my teeth are at that time, and the temperature of the room.
