With my health insurance...the hopsital claimed my insurance denied the bill. My insurance company had never had the bill submitted, so really it wasn't their fault. They fixed it too with a level of civility that I could not.

I've had trouble GETTING it. But when I was 18, I had to see a cardiologist and it took him a bit to find me a heart monitor that my insurance would cover.

Yes, I got screwed for $700 at the dentist since they decided to do things that werent covered, then sent creditors after me when I moved to another city even though I went to the office 2-3 times after that money was owed and they never said a word.

I used to have a lot of trouble with an insurance company that my ex-husband's employer had switched over to --it was 'dedicated' to the construction industry. They would not cover many things and hid them with double talk and small print. This company has since settled class action lawsuit against them to the advantage of those suing them.

The employer went back to the earlier company and I only had some minor issues with dental and emergency visits.

Oh well not in that sense, but for a period of time Walgreens wasn't taking expressscripts through bluecross. Which was annoying

Yes, in the sense I don't have any but have monthly reoccurring medical bills.

Sadly, it's still cheaper for me to just pay the bills than it is for me to actually get and pay for insurance, especially since insurance will only cover part of the costs.
