It's funny you ask now. Both my partner and I have birthdays in early spring. I always ask him if he wants something special, but he looks at me deadpan and goes "We do it. Already." In his mind, birthday sex isn't that special since we fulfill all our kinks (whenever they strike) at this point. For us, birthday sex is used to pick the tone of the sexy times.

I, too, wouldn't do something sexual for a special occasion that isn't normally okay with me. I WOULD and DO plan special surprises for her, but they don't involve anything that would make me uncomfortable, although they might involve a number of things I don't get to do very often. For example, if she really wanted to penetrate me anally and I wasn't okay with that, I wouldn't do it just because it was her birthday. However, I WOULD wear especially nice lingerie and do all her/our favorite things, including some new stuff I know she'll like.

We don't do anything sexually special for each other. Sex is just like any other day. If we are going to try something new I sure wouldn't want it on a special occassion just in case it turned out horrible.

Yes. Very, very special! I like to add fun, unexpected touches when there's a special day.

As for "sexual wishes" that I'd usually say no to? None of that. If it's something either of us would say no to doing any other day of the year, then it's not a good idea to decide to go through with it because of it being a special day. Only if it's something we both want to do, we'll do it. If not, then it's not something we do.
