I've used one, or, rather, had one used on me. It was hard to keep it in place... the side against me got very slippery and it was a struggle to keep it not moving while my partner was going down on me. Never tried it with toys. We used one and never again, it was frustrating and we didn't really need it.

I am sure they are useful if needed, but they seem quite cumbersome. If there is concern, I would just skip that part of sex and do other things with other forms of protection.
