The form was a points reconsideration form available for a limited time right after the big announcement on 6/19 that we could only use 15% of our points on any orders. In response to requests, Fred offered the opportunity to submit the form which was our agreement that EF would go through all of our activity on the site and determine if we had ever committed points abuse/point farming. If we were found guilty, then we would lose all points accumulated before 6/19. However, if we are cleared and approved, then we would be able to use those points from before the 19th at the previous rate of 100%. Many people decided not to submit the form, but I think there still were a lot who did, thus the long time it took for them to go through all those accounts. Points gathered after 6/19 were not affected and are still available at the 15% rate. If someone did not submit the form, they got to keep whatever points they had, they just can only use them at 15%, same as the ones we accumulate now.
I hope that explains it? I think I stuck to the facts and hope it is OK to post this comment? I know a lot of people will continue to have the same question as mailroomorder, so I think having it spelled out somewhere is probably a good thing.
