But yeah I would say ones with no holes would mean less air passing through your mouth, if no air that way at all. Meaning ya would have to breath through your nose!

One other big difference that hasn't been mentioned: gags with holes (or wiffle ball gags) have a tendency to cause a lot of drooling while regular ball gags do not. That's either a good thing or a bad thing depending on your preferences--some people prefer wiffle ball gags precisely because of the increased drooling--but it's definitely why I prefer regular over wiffle ball gags.
